Insurance and coverage

Utah law

Utah requires many insurers to cover IVF (law effective 2021) -- public-employee pilot ($4,000 indemnity); Medicaid genetic-condition IVF. (We are not sure how this law applies. Check with your insurer.)

Utah law requires coverage of fertility preservation before treatment that can cause infertility, such as cancer treatment (effective 2021).

Accepted insurance

Check your own plan's IVF benefit; accepting an insurer does not mean every plan covers IVF.
