Insurance and coverage

Hawaii law

Hawaii requires many insurers to cover IVF (law effective 1987) -- 1 IVF cycle (one-time benefit). (We are not sure how this law applies. Check with your insurer.)

Hawaii law requires coverage of fertility preservation before treatment that can cause infertility, such as cancer treatment (effective 2027).

Citation: HB 1864 (signed 2026-07-09); distinct from the older, narrower IVF-benefit mandate at Haw. Rev. Stat. S431:10A-116.5

Accepted insurance

Check your own plan's IVF benefit; accepting an insurer does not mean every plan covers IVF.

What insurers pay this clinic

These are rates insurers have agreed to pay the clinic. They are not your cost and are not part of the prices above.

  • CignaIVF case rate: $3,907 to $4,842
  • UnitedHealthcareIVF case rate: $6,314 to $7,428

A negotiated rate is what the insurer has agreed to pay the clinic for a billed service, not the patient's cost; the patient's share depends on the plan's IVF benefit, deductible and coinsurance. A clinic in the insurer's network file with no IVF billing codes suggests the plan likely excludes IVF.
